This position is based in a facility licensed by the Community Care Licensing (CCL) Division of the Department of Social Services in the state of CA. All employees of this facility must clear a livescan background check. Any person with previous convictions at any point in their lives, will not receive a clearance from CCL. This includes all convictions: felonies, misdemeanors and DUIs.
Previous convictions are not an automatic bar to employment at other non licensed Larkin Street Youth Services sites. Each case will be considered on its own merit.
SCOPE OF SERVICES
Assisted Care is a 12 bed residential RCF-CI (licensed) facility for youth (18-24) living with HIV/AIDS. After Care provides housing subsidies, case management, and support services to youth (18-24) housed in the community who are living with HIV/AIDS. The programs offer comprehensive psycho-social, educational, vocational, and employment opportunities to the participants served to ensure their successful transition into greater independence and self sufficiency. There is an emphasis on serving young adults with behavioral health and substance abuse needs.
The primary role of the Contract Nutritionist is to provide nutritional consultation to the staff and clients of the program. The nutritionist will work with the program’s Food Services Coordinator around menu planning to ensure that the food program is meeting the nutritional needs of clients with HIV. The nutritionist will also offer one on one nutritional screening, counseling and nutritional planning to clients and may offer a nutrition focused client group. The nutritionist will also provide some training to program staff on the nutritional needs of clients with HIV.
DUTIES AND RESPONSIBILITIES
- Individual nutritional assessments, counseling and education
- Provide trainings to HIV Services program staff
- consultation to Food Services Coordinator
- Working closely with members of the Larkin Street team, including case managers, residential counselors and program managers to insure coordinated and effective services.
QUALIFICATIONS
- Degree in Nutrition. license required
- 2-3 years of experience with providing nutritional counseling to an high-risk HIV positive population.
- 2-3 years experience working with youth and/or young adults
- Working knowledge of issues facing homeless and LGBTQ youth.
- Proficient in Microsoft Office (Word, Excel, and Outlook)
